Please read the enclosed information to determine where to send all required copies of missing documents .
Please include a copy of this letter with your correspondence and write your case number clearly (exactly as it appears below) on the upper right-hand corner of all documents.
if a period of one year passes without contacting NVC (by TEL orMAIL),all submitted fees and documents will expire .in this case,fees and documents will need to be resubmitted in order to continue the immigration process.

If you have not already done so,please submit a completed copy of form DS-230 Part I ,Application for Immigrant visa and Alien Registration - biographic Data,for this applicant .You must answer each question completely and NOT skip any questions ;questions that are Not Applicable should be so marked.please pay specific attention to the following questions:
1)--Provide your current nationality and place of birth.
2)--Provide your current marital status and number of times married.copies of divorce and death certificates pertaining to previous marriages must also be submitted.
3)--list the place(s) that you have resided for at least six months since you reached the age of sixteen.If a gap exists in your residence history of more than six months,please write a note indicating the countries in which you resided during that period.
4--Provide the dates and your vise status for all previous visits to the U.S.
We have enclosed a blank copy of the form .your visa applicaton cannot be processed until this form is complete .

4--提供你以前所有访问美国的日期及签证状况.
